The questionnaire is mostly for those who have ordered a DNA genetic test kit —it is not necessary to complete if you are doing a Covid Test.

Please note that the questionnaire is a carousel on your main dashboard so it will take you to the beginning of the questions if you continue clicking on "Skip" — simply ignore the questionnaire for now, unless you wish to have your generic questions placed on your final covid report.
